Spring Mobile Immunization Clinic

Dear 6th Grade Families,
 
All students entering the 7th grade in the Fall 2021 are required to be up to date on their immunizations.  This includes the required Tetanus, Diphtheria, Pertussis (Tdap) vaccine and Meningococcal (MCV4) vaccine.  In an effort to assist families with meeting this requirement, Fairfield Community Health Center has partnered with Lancaster City Schools to provide an after school on site vaccination clinic at Lancaster High School on Monday May 17, 2021 from 4:30pm-8:00pm.  The on site vaccination clinic will be located in front of the main entrance to Lancaster High School.  
 
 *Parent/Guardian MUST be present with their child at the mobile immunization clinic at Lancaster High School unless they are 18 years old or older.
 

To register your child for the mobile immunization clinic, you will need to complete the following paperwork listed below and return all required forms to your child’s school by Friday, April 30, 2021:

Student’s Vaccination Record

A most recent copy of the Student’s Insurance Card/Cards (front and back)

Completed and Signed Registration/Consent Form

Attached is a cover letter with greater detail regarding the upcoming mobile immunization clinic as well as the required registration and consent from.
