Lancaster City Schools is excited to announce our partnership with the Fairfield Community Health Center to provide back to school immunization clinics.   We will be offering immunizations that are required for school attendance. The Fairfield Community Health Center plans to administer the vaccines at 3 different school locations in the evening after school and will be sending more specific information soon.  If you are interested in participating, please complete all the steps listed below and return all the required information to your child's school by Friday October 9, 2020.
Current Copy of Immunization Record
Copy of Child's Insurance Card (Front/Back)  
Completed and Signed Registration and Consent Form
Risk Assessment Form

If you have any questions about the vaccines or vaccination clinics, please call 740-277-6043 on weekdays from 8 AM to 5 PM.  For more information, you can visit the CDC’s website at regarding school age immunizations, and to view the Vaccine Information Sheet.  Your child’s health care provider also can answer your questions about the immunizations available at the school clinic as well as if they are able to provide the immunizations to your child should you choose not to participate in the immunization clinics offered by Fairfield Community Health Center.
